Bioidentical Hormone Therapy

Bioidentical hormone replacement therapy (BHRT) uses hormones that are chemically identical to those naturally produced by the human body, primarily estrogen, progesterone, and testosterone. Derived from plant sources like yams and soy, these hormones are modified in laboratories to match human hormones at the molecular level. The body metabolizes bioidentical hormones the same way as endogenous hormones, which may result in better tolerability and fewer side effects compared to synthetic alternatives. BHRT can be administered through creams, gels, patches, pellets, or oral preparations, with each method offering different absorption rates and blood level consistency.

BHRT may be appropriate for women experiencing perimenopausal or menopausal symptoms such as hot flashes, night sweats, mood changes, sleep disturbances, or vaginal dryness. Men with clinically diagnosed low testosterone accompanied by fatigue, decreased libido, mood changes, or loss of muscle mass may also be candidates, as well as younger individuals with premature ovarian insufficiency or surgical menopause. However, BHRT is not suitable for everyone—those with a history of hormone-sensitive cancers, blood clotting disorders, unexplained vaginal bleeding, or certain cardiovascular conditions require careful evaluation or may be advised against hormone therapy.

My prescribing procedure begins with a comprehensive consultation to review your medical history, symptoms, and treatment goals, followed by baseline laboratory testing of hormone levels and other relevant markers. Based on these results, I develop an individualized treatment plan specifying the appropriate hormones, dosages, and delivery methods for your needs. I typically start with conservative doses and schedule follow-up at 6-8 weeks to assess response and adjust as needed. Ongoing monitoring occurs every 3-6 months initially, then annually once stable, ensuring optimal hormone levels while minimizing risks.
